This section contains resources for Survey Vendors to administer the CAHPS Hospice Survey.
Survey vendors must format their data submissions to the CAHPS Hospice Survey Data Warehouse following the CAHPS Hospice Survey XML File Specifications.
This section contains links to the forms that survey vendors must use if they need to report discrepancies in following standard CAHPS Hospice Survey protocols, request exceptions for certain variations from protocols, obtain data warehouse access, apply for approval for participation (during the participation period in early spring), and register for training (in mid-summer).

The training materials (agendas and training slides) for the September 25, 2025 CAHPS Hospice Survey Vendor Training session are available.
The CAHPS Hospice Survey Participation Form for Survey Vendors is available beginning on June 4, 2026 through June 25, 2026.
Please note, survey vendors that are currently approved to administer the CAHPS Hospice Survey are not required to submit a CAHPS Hospice Survey Participation Form (unless the survey vendor is requesting to be approved for additional modes of survey administration or the survey vendor does not have any contracted hospice clients and it has been two years since the survey vendor received approval to administer the CAHPS Hospice Survey).
